How to Fill out the Form

Only an Admin User can access and fill out this form. Only one User has to fill out the form for an entire company.
NOTE: It is extremely important to complete the form as accurately as possible. Anything omitted or incorrect, even typos, will result in a rejection. The only way to resolve a rejection is to submit a new form, which starts the clock all over. At present, new registrations are taking between 2-10 days to approve.
Company Name: Enter your legal company name exactly as it appear on your tax returns. If you are sole proprietor with no formal company name, enter your full name. Do not include any punctuation, like commas, apostrophes, or dashes.
Entity Type: This is the type of company you are. Most RingRx customers are a Private Company. If you have staff and have filed as an LLC, an S Corp, a C Corp, or any other form of company, you are a Private Company. If you are a non-profit, Government, or Publicly Traded Company, then you should select those.
EIN: This is your Employee ID Number and is provided to you by the IRS for tax filing purposes. If you are a sole proprietor use your own Social Security Number. It is critically important that the name listed in Company Name match the EIN or SSN. If these do not match, your registration will be rejected.
EIN Issuing Country: This defaults to the USA and there are no options for other countries at this time
Vertical: This is the industry you are in, mostly likely Healthcare.
First and Last Name: Provide the First and Last legal name of the owner of the RingRx account. Be VERY cautious with this and do not enter anything other than your legal first and last name that appears on legal documents.
Address Info: Provide the physical location of your business. This should be the same address that RingRx has on file for your account.
Website: If you have a website, it is a critically important component of this registration. It is a required field, so you MUST put something here. Even if it is a small one page website, a profile on a professional organization (like Psychology Today), or even a social media profile, you must put something. If you are not able to include your terms and conditions, privacy policy or consent to text form (also called the ”Call-to-Action”) you must state in this field that the reviewer must look at the paper copies you have provided with these materials.
- If you have a website, do not enter a social medial profile link. Use the website, as it is the easiest way to vet your company.
- Make sure it’s a valid URL – the address that pulls up your site. If you are unsure what a URL is or how to properly format it, please get help from someone who does. The URL will look something like: www.acmemedicalgroup.com.
- If you do not have a website or something you can enter here, you must include all of the required documentation that is usually on a website in paper formats (terms and conditions, privacy policy and call to action (consent) form). The website field should say “Please see all paper forms.”
- No SHAFT-C content on your website. No images, text, buttons, ads, or messaging related to Sex, Hate, Alcohol, Firearms, Tobacco, or Cannabis (or CBD or any other prohibited or controlled substance, including Ketamine).
- Opt-in and opt-out consent information should be displayed clearly and conspicuously on the website or provided in a paper form.
Sample Message #1: We have prefilled some content that works well for healthcare providers. If this does not make sense in your case, feel free to enter something more appropriate. Keep it brief and clear. The following examples adhere to standards that have been advised by the registrar.

- Good evening Church family, we are having an in-person worship service tomorrow at 10:30 am. We will also be streaming the service over Facebook Live for those unable to attend. Reminder: tomorrow is the day to bring the donation boxes.
- Hi %FirstName%! This is Ramiro with Bandwidth. We’d love to invite you to visit our booth at the upcoming Conference, which is taking place virtually and in person from Nov 9-13! Tickets are available now. There will be panels relating to voice, messaging, and 911! Register at www.bandwidth.com. Will we see you at the conference this year?

Description: What will you be using texting for? It is for basic communications with customers, as opposed to large campaigns for marketing, customers support, or sales. Unless you have very specific needs for higher volume texting, this is the most appropriate use. Prefilled description has worked well, but if its not appropriate, enter your own information as you see fit.
Message Flow: State briefly and clearly how messages initiate with a patient/client, and why that relationship is started. We prefilled something that works for most healthcare providers, but if it is not appropriate, then feel free to create your own. This should explain why you are texting with the patient and how you get their number.
